NDIS Support Coordination
Barwon Child, Youth & Family provide for the coordination of supports as defined in the plans of participants of the NDIS, from the ages 6-65 yrs.
There are three levels of coordination:
- Support Connection- assisting participants to connect with services and support the capacity of people to manage their own supports.
- Support Coordination- assisting with service connection in a complex environment involving crisis management and developing capacity in the participant and their networks.
- Specialist Support Coordination – Support Coordination within a therapeutic framework necessitated by a high level of risk.
Being a registered NDIS service provider allows BCYF to engage with and effectively support people of all abilities to participate in their community. BCYF also works with other disability and mainstream service providers to support people’s access to the NDIS.
Support Coordination compliments our Family, Youth, Early Years and Specialist Intervention Services, providing an integrated, disability informed community service platform in the Barwon and South-West regions.
